How is the shoe hanger fixed to the wall?

With the screw and wall plug in the box. It is screw fixed only, no adhesive version, so it holds a pair of shoes without shifting. Works in plasterboard with the supplied plug, or in brick and block with a masonry plug.

What is in the box and how does it go together?

Four parts: a wall plate, an angled arm, and two shoe holders. The plate screws to the wall, the arm screws into the plate, and the two holders slide onto the arm. Fixings are included. A couple of minutes with a drill and a screwdriver.

What shoes does it fit?

Cycling shoes of any kind: road, MTB and gravel. It also holds trainers, running shoes and most everyday shoes. Shoes hang from the heel, so nothing presses on the sole or the upper.

Can I use several to make a wall shoe rack?

Yes. Each hanger holds one pair. Fit a row of them, one per pair, and you have a wall mounted shoe rack that uses no floor space and lets you see every pair.
